Medical Records and Documentation:

Ages 21 and over -
Medical records ARE required for the appointment, please call us to verify your qualifying condition prior to your appointment. You must have a qualifying condition under RCW 69.51A.

Ages 18-20 -
Medical records and documentation required. Your records and documents must outline a qualifying condition under RCW 69.51A.

Ages 17 and under -
Legal guardian attendance and medical records required. Records and documents must outline a qualifying condition under RCW 69.51A.

A Washington State ID/Driver’s License is required for the appointment. If you’re not a resident and have the intent to become a resident but don’t yet have a Washington State ID, you can bring in your current government ID and YOU NEED: (2 of the 3)

A government document with your full name and Washington State Address

Qualifying conditions to receive your MMJ Authorization:

A. Intractable pain, chronic pain limited for the purpose of this chapter to mean pain unrelieved by standard medical treatments and medications;

D. Crohn’s disease with debilitating symptoms unrelieved by standard treatments or medications;

B. Cancer, human immunodeficiency virus (HIV), multiple sclerosis, epilepsy or other seizure disorder, or spasticity disorders;

E. Hepatitis C with debilitating nausea

C. Glaucoma, either acute or chronic, limited for the purpose of this chapter to mean increased intraocular pressure unrelieved by standard treatments and medications;

F. Diseases, including anorexia, which result in nausea, vomiting, wasting, appetite loss, cramping, seizures, muscle spasms, or spasticity, when these symptoms are unrelieved by standard treatments or medications.
